Forma gives you a clear path for connecting with your children and coaching them through the skills and virtues that matter most.

How It Works

Welcome to Forma! To get started, go to the Skills Tracker and add a child using the + Add a Child button. Then explore the four sections:

  • Skills Tracker — log conversations and demonstrations for each child
  • Family Meetings — record your family meetings
  • Parent Goals — track your personal goals as a parent
  • Planning — assign dates and schedule upcoming conversations

Daily Habits Tracker
Track each child’s daily household habits right from the Skills Tracker. Each child gets 3 habit slots you can customize and swap as habits become second nature.
Conversations Per Topic
The default is one conversation per topic. Select two or three if you’d like the option to check off repeat conversations — for example, revisiting a topic at a different age or going deeper the second time.
Daily Reminder
Set a daily calendar reminder to visit Forma. Observe your children, check off skills, add notes, and schedule conversations or meetings.
The Apple Calendar download (.ics file) also works with any other calendar app.

Data Encryption
Optionally encrypt your family data with your password. Your data can be un-encrypted here with one click. Warning: if you are logged out and forget your password, encrypted data cannot be recovered.

Terms of Use

Forma is a product of Friends of Aquinas LLC, a Minnesota limited liability company. By creating an account or using Forma, you agree to these Terms of Use.

1. What Forma Is. Forma is a family formation tracking tool. It provides conversation prompts, skill-tracking features, and organizational tools. Forma does not provide professional advice in education, psychology, medicine, or any other field. All content is for informational and personal use only.

2. Personal & Family Use Only. Your Forma account is for your family’s personal use. You may not resell, redistribute, or share your account credentials outside your household.

3. Payment & No Refunds. Forma is available as a one-time purchase. All sales are final. No refunds will be issued. A free trial is provided so you can evaluate the product before purchasing. By completing your purchase, you acknowledge that you have had the opportunity to try Forma and are satisfied with the product.

4. Your Data. By default, your data (children, progress, notes, meetings, goals) is stored in a very unreadable form using Google Sheets via a secure service account. We do not sell or share your data with third parties. You have the option of fully encrypting (and un-encrypting) your data in the Settings screen, but it will then be attached to your password so if you forget your password, your data will be lost. You may download your data at any time at the Settings screen. You may request deletion of your data at any time by emailing sally@friendsofaquinas.com.

5. Content & Intellectual Property. All content within Forma — including conversation topics, lesson guides, quizzes, and app design — is the intellectual property of Friends of Aquinas LLC. You may not copy, reproduce, or distribute this content without written permission.

6. No Warranty. Forma is provided “as is” without warranty of any kind. We do our best to keep the app running smoothly, but we do not guarantee uninterrupted access. We are not liable for any data loss, though we take reasonable steps to protect your information.

7. Limitation of Liability. To the maximum extent permitted by law, Friends of Aquinas LLC shall not be liable for any indirect, incidental, or consequential damages arising from your use of Forma. Our total liability shall not exceed the amount you paid for the product.

8. Changes to Terms. We may update these terms from time to time. Continued use of Forma after changes constitutes acceptance of the updated terms.
